Why should I confirm the department first?

Large insurers use different fax numbers for claims, appeals, prior authorization, medical records, pharmacy matters, and employer plans. A number that belongs to the right company can still route to the wrong team.

What are the steps?

  1. 1Identify the task, deadline, plan, member, claim or appeal number, and requested documents.
  2. 2Confirm the department’s fax number using the current member card, authenticated portal, official form, or a representative reached through an official phone number.
  3. 3Ask whether a specific cover sheet, form, barcode, page limit, or attention line is required.
  4. 4Send copies of only the requested records in a clearly labeled order.
  5. 5Save the transmission result and follow the insurer’s process to confirm the documents were attached to the right case.

What belongs on the cover sheet?

  • Department or named recipient
  • Verified fax number
  • Your name and safe callback number
  • Member, claim, appeal, or authorization reference requested by the insurer
  • A neutral document description and total page count

Note: Do not place more medical or financial detail on the cover sheet than the recipient needs to route the fax.

How do I know the insurer received it?

A delivery result indicates that the receiving fax system accepted the transmission; it does not prove that staff attached or reviewed it. Use the insurer’s official status or follow-up process, especially before a deadline.
